Privacy, in plain words.
What we ask for, why we ask, where it goes, and how to ask us about it.
What the booking form collects
The form asks for four things:
- Which door you're coming through (Hers: menopause HRT, His: men's hormone health, or not sure yet)
- Your name
- Your phone number or email
- The best time to reach you
There's also a separate box, "Send me occasional news from Hormone Therapy Ontario". It's never ticked for you, and leaving it empty doesn't change your booking.
That's all. The form doesn't ask for medical details.
Details you send through this website are stored and processed by KremCRM, the customer relationship system we use, on Microsoft Azure servers in Israel, outside Canada.
Your symptom list
The list page keeps your ticks in this browser tab only, until you close it. Nothing you tick is sent to us. You bring the list to your assessment, printed or on your phone. "Clear my list" removes it from the device, and clearing your browser data removes it too.
What we use your details for
- To call or email you back and book your assessment
- To send you occasional news from Hormone Therapy Ontario, only if you ticked that box. You can unsubscribe at any time.
Site statistics
Every page on this site loads Google Analytics 4, through Google Tag Manager, to measure how the site is used: which pages are viewed and how many visits there are. It sets cookies whose names start with _ga, holding a random identifier, so that repeat visits can be counted. It's for measurement only. No health information, nothing you tick on your symptom list and nothing you type into the booking form is sent to it. Google Analytics is provided by Google LLC, which may process this data outside Canada. To block it, turn off cookies for this site in your browser settings, or install Google's Analytics opt-out browser add-on.
Asking about, correcting or deleting your details
You can ask what details we hold about you, ask us to correct them, or ask us to delete them. Email our privacy contact at privacy@hormonetherapyontario.ca.
Urgent health concerns
The booking form isn't watched for emergencies. In an emergency, call 911.
