What comes out in the room
Your list.Tick what's been happening. We'll go through every line at your assessment.
A perimenopause and menopause symptoms checklist, plus five lines for men, in your words, that prints on one page.
Tick the lines that sound like you, mark how often, then print one page or keep this tab open on your phone. Two minutes. Nothing you tick leaves this device.
Heat and sleep 4 lines
Hot flashes and night sweats are the symptoms most people know. Broken sleep often comes with them.
Mind and mood 4 lines
Often the most confusing, because they don't sound like hormones at all.
Cycle and body 6 lines
Changes in your period are often the first clear sign of perimenopause.
Sex and bladder 3 lines
The ones people find hardest to say out loud. That's exactly why they belong on a list.
The five-line list 5 lines
Low energy and a low sex drive have many causes. If these sound familiar, the next step is an assessment and morning bloodwork, not a guess.
The 3am symptoms are gone by the 3pm appointment.
- Symptoms come and go. A list doesn't forget.
- Your words say more. "I lose words mid-sentence" tells the NP more than a score out of ten.
- Your assessment starts from it. We work down it together, the line that bothers you most first.
What the list is, and isn't
It isn't a test, and it can't tell you whether you're in perimenopause or what's behind a symptom. Many of these lines have other causes, from iron levels to stress, which is why an assessment matters. What the list does is make sure nothing gets left out of the conversation.
Ontario's menopause quality standard asks clinicians to ask about menopause symptoms from age 40. If nobody asked you, we will.

Bring it to your assessment
- Your printed list, or this page open on your phone
- Your medications and supplements
- Any recent bloodwork
- The date of your last period, if it applies
Some things don't wait for a list. Bleeding after menopause, or a new breast lump: contact a clinician promptly or call Health811 (811). Chest pain, sudden weakness or stroke signs: call 911.
